Immediate Home Care, Emergency Care No Matter Where You Are

We are available to start caring for your loved one immediately. Even if you are unavailable to meet in person to sign paperwork on behalf of your loved one, we can assist you. You can expect a seamless discharge, even if it is from a hospital or facility. Any paperwork can be completed through email or fax if your loved one cannot do so.

Planning the Right Senior In-Home Care Plan

At the time of your free personal consultation, a Golden Years Care Manager will:

• Discuss all necessary care needs
• Create a specialized care plan using our exclusive Co-Active Care System
• Work with you or your loved one to ensure the care plan accommodates your schedule, supports your desires, and meets your care requirements.
• Coordinate billing with insurance providers if needed
• Assign a personal Care Manager

Designing a Comprehensive Care plan

Next, our Care Team selects the most compatible caregiver based on necessary experience and personality to meet your care requirements. Then we invite the caregiver to your home to meet you so you can participate in the selection process. Once services are initiated, we will implement our specialized care plan using our exclusive Co- Active Care system. This system involves input from you, your loved one, the caregiver and their supervisor. This is partially inspired by medical patient research.

Care Progress Notes and Reviews

We can make available to you daily progress notes concerning your loved one’s daily activities. Upon request, a 30 day care evaluation conducted by our Nurse Supervisor can be provided. This ensures you have complete assurance in progress and top care.
